ACP Journal Club. Review: epidural corticosteroids reduce short- but not long-term leg pain and disability in sciatica.

چکیده

Review scope Included studies compared epidural corticosteroid injections with placebo (administration of an inert or innocuous substance into the epidural space or adjacent spinal tissue) in patients with sciatica. Studies had to report ≥ 1 of overall pain intensity, leg pain intensity, back pain intensity, or disability status. Corticosteroids could be delivered into the epidural space by any of the caudal, interlaminar, or transforaminal approaches. Studies that included patients with foraminal stenosis or lateral recess stenosis were included, as were studies with mixed populations if data for subgroups with sciatica could be identified. Studies of patients who previously had surgery or had sciatica due to spinal canal stenosis were excluded.

The Value of Short-Term Pain Relief in Predicting the Long-Term Outcome of Lumbar Transforaminal Epidural Steroid Injections.

BACKGROUND A previous report demonstrated predictive power of short-term leg pain relief after lumbar transforaminal epidural steroid injections for 1-month treatment response. The question whether the long-term response could be similarly predicted remained unanswered. METHODS A prospective cohort of 57 patients who underwent a transforaminal epidural steroid injection for sciatica secondary...

Systematic review of therapeutic lumbar transforaminal epidural steroid injections.

BACKGROUND Epidural injection of corticosteroids is one of the most commonly used interventions in managing chronic spinal pain. The transforaminal route to the lumbar epidural space for steroid injection has gained rapid and widespread acceptance for the treatment of lumbar and leg pain. However, there are few well-designed randomized, controlled studies to determine the effectiveness of epidu...

The Epidural Treatment of Sciatica: Its Origin and Evolution.

Epidural injection with corticosteroids is a common treatment option for patients with lower back pain or sciatica. In this paper we review its origin and evolution. The first injections were given around 1900 in Paris by Jean Sicard (1872-1929) and Fernand Cathelin (1873-1945), who worked independently. They both injected small volumes of cocaine into the sacral hiatus. After a slow start, the...

Epidural neuroplasty versus physiotherapy to relieve pain in patients with sciatica: a prospective randomized blinded clinical trial.

BACKGROUND Epidural neuroplasty seems to be one of the promising minimally invasive techniques for adhesiolysis in patients with chronic sciatica with or without low back pain. However, because no data exist from randomized studies the aim was to investigate whether this procedure is superior to conservative treatment with physiotherapy. METHODS A total of 99 patients with chronic low back pa...
